Should be "(Demo) (Kiosk)", not "(Beta). It is from the GameCube software "Interactive Multi-Game Demo Disc - May 2004 (Japan)".
It has "sample" in the name in the ROM header, and has demo features such as a "demo finished" screen: https://tcrf.net/images/0/0c/KirbyAmazingMirror-DemoCompletion.png , and limited access to locations.
