I think there is no need to add "for Game Boy" text to title so we'll have just simple title:
Bases Loaded (USA)

"for Game Boy" is printed on the box. On the title screen of the game there is a "GB" to be seen on the right upper side of "Loaded". So it should be read as "Bases Loaded GB". In my opinion the title "Bases Loaded for Game Boy" is appropriate and could stay like this.

The intro text says "Thank you for purchasing JALECO™'S Bases Loaded® Game Pak for your Nintendo® Game Boy™"
But I think this is still unclear. So I'm leaving this open.

The "for Game Boy" is pretty small on the box, as is the "GB" on the title screen, and the back of the box also just refers to it as "Bases Loaded". I think removing "for Game Boy" is probably fine - its not necessarily part of the "actual" title.
